Due to unforeseen changes in the Hawaiʻi market and unresolved contract negotiations between MDX and Humana, Pukalani Family Practice may no longer participate with Humana insurance plans effective March 1, 2026.

Your Options
1. Continue Care as a Non-Participating Provider (Humana PPO Only)
Patients with a Humana PPO plan may still be able to see Pukalani Family Practice as a non-participating provider. Coverage and out-of-pocket costs vary by plan, so we strongly encourage you to contact Humana directly to verify your benefits.
Please note: Patients enrolled in a Humana HMO plan will not be able to see us as a non-participating provider.
2. Change Insurance Plans
