Robert Mastroianni, MD

Graduated from Tufts University Medical School in Boston in 1977.  He practiced Emergency Medicine and Family Practice on the west coast, primarily in the Sierra Nevada's until moving to Maui in 2002. He counts his years at Pukalani Family Practice in association with his caring and skilled staff as some of the finest in his life. He feels truly blessed as part of this 'Ohana and these Paradise Islands.

Jacob Wardach, A-GNP, NP-C

Is a nurse practitioner at Pukalani Family Practice born and raised in Detroit, Michigan. Jake received his Bachelor’s of Science in Nursing from the University of Michigan in 2014 and practiced as an RN and traveling nurse throughout the US including Maui, the island that he and his wife now love to call home. He received his Master’s in Nursing in Adult Gerontology Nurse Practitioner from Spring Arbor University in 2018. In his spare time, Jake likes to enjoy the outdoor paradise of Maui through surfing, hiking, camping, paddling, and playing as many sports as possible. He is inspired by the dedicated staff and patients at Pukalani Family Practice and is passionate about helping others achieve their individualized state of health.

Liat Miller, APRN

Aloha! My name is Liat Miller. I am grateful to call Maui my home and I am committed to the health and safety of our community. I was born and raised in Israel where I served as an Air Force Officer. Shortly after my service, I graduated from Bar-Ilan University with an undergraduate degree in Biotechnology. Continuing my education at the University of Hawai’i at Manoa, I earned my second undergraduate degree in Nursing as well as my Masters in Nursing as a Family Nurse Practitioner. In my spare time, I enjoy surfing, yoga, canoe paddling and hanging out with my two dogs. I am passionate about whole-plant-based diet as I strongly believe in its power to improve and prevent chronic diseases such as obesity, diabetes, heart diseases and more. While working at the Pukalani Family Practice with our great Ohana, I aspire to empower individuals and families to improve their lives with high quality healthcare.

Lisa Herrmann APRN

Aloha my name is Lisa Herrmann, Certified Family Nurse Practitioner, I have been involved in the healthcare industry for many years.  As a personal trainer for more than 20 years, a sports nutritionist, and a certified active isolated stretching coach.  While on the mainland I received my BSN, then pursued my MSN and my FNP.  As an RN I worked in home health, wound care, diabetic care, urgent care and as a school nurse.  If I’m not working at Pukalani Family Practice, you can find me working in the ER.  As a PCP, my focus is on holistic care of my patients, treating not only the symptoms but the person as whole.   In my free time you can find me at the beach with my two kids and husband, at the gym, riding my bike, running or cooking whole foods plant based meals.  

Ann Craddock, MSN, APRN, FNP

 It has been a privilege providing services to the community in Maui as a Nurse Practitioner since 2004. I am originally from Northern California and moved to Maui in 2001. I worked as an RN at Kaiser Permanente and Planned Parenthood of Hawaii then continued my education receiving my Women’s Health Nurse Practitioner (WHNP) certification via Planned Parenthood of MN/SD in Minnesota (in collaboration with the University of Minnesota) in 2004. After working as a WHNP specializing in gynecological care until 2014, I returned to school obtaining my Family Nurse Practitioner (FNP) certification, and Master of Science in Nursing Degree from St. Joseph’s College of Maine. When I’m not working I enjoy surfing, hiking, and spending time with my husband Kevin, and two dogs Kaila and Lani.
